Procard Transmittal Form Submit this form to the buisness office after any procard purchase.
Purchase Request Form Submit this form to the business office for any purchase made which involves an invoice being received. This form should have the appropriate account number to pay the invoice and an approval signature.

Folder Request Form Request folders and/or one-pagers from Communications printed on glossy stock.
Powerpoint Template For Texas A&M faculty to use when creating powerpoints. To customize template, edit text for the footer. This will change text "College of Science" on master slides.
Promotional Item Request Form Internal requests for promotional stock items.
Public Relations Request To submit a story idea.
Talent Release Form Please have any folks involved in any non-public university photography/video, usually for promotion purposes, sign these in advance. This allows use for marketing pieces on behalf of the university.

Faculty Development Leave University guidelines and application forms for Faculty Development Leave. This program, often referred to as sabbatical leave at other institutions, is for faculty wishing to seek redirection in their careers, as well as to advance or complete an existing project or line of work. Tenured faculty with five years of service may apply and receive full pay for one semester or half-pay for two semesters.
Faculty Grievance Procedures Grievance procedures for any faculty member believing that he/she has cause for grievance concerning, but not limited to: working conditions, salary inequities or teaching assignments. These procedures do not apply to: constitutional rights and academic freedom, tenure decisions, sexual harassment or dismissal; there are separate avenues for appeal of these.

Outstanding Staff Achievement Award Nomination Form To nominate staff in recognition for their dedication, enthusiasm, accomplishments, and contributions. Award winners receive $500 and a plaque of appreciation.
Promotion and Tenure Guidelines University guidelines for annual submission of tenure and promotion recommendations for use by faculty eligible for tenure and promotion, department committees, and university administrators.
Staff Issue Submission Form To submit an issue to the University Staff Council, you can contact your respective representative by mail an Issue Submission Form. Please NOTE: The University Staff Council will not act in an advocacy role in any specific employee/employer policy matter or dispute within the University, nor will it assume the role of arbitrator in any such matter. Employees are encouraged to work directly with their supervisor or management personnel, or their appropriate Human Resources office, if issues are specific to their personal situation.
Strategic Plan 2011 This document is the result of much discussion and planning over the past two years (starting with the Academic Master Planning Process in mid-2008) and consists of the Strategic Plan for the College of Science for 2011-2015.

Q-Drop Form For students who wish to drop a course after add/drop date and within the Q-drop deadline. All students are limited to a maximum of 3 Q-drops.
